A grievance form  is available for your convenience.
If you choose not to use the form, your written grievance should include, at a minimum:
• Your name
• Your address
• Your contact information (email, TTY, telephone number, mailing address, as appropriate)
• The date of the complaint
• A description of the problem you encountered;
• A possible solution or desired resolution.

Title II of the ADA specifically prohibits state and local governments from discriminating against persons with disabilities or from excluding participation in or denying benefits of programs, services or activities to persons with disabilities. The Americans with Disabilities Act (“ADA”) enacted on July 26, 1990, and later amended, provides comprehensive civil rights protections to persons with disabilities.

The City of Bradenton complies with the requirements set forth under Title II of the ADA with regard to its services, programs and activities and does not discriminate against individuals with disabilities.

Title VI of the Civil Rights Act of 1964 provides that no person in the United States shall, on the grounds of race, color, national origin (including Limited English Proficiency), or sex, be excluded from participation in, be denied the benefits of, or otherwise be subjected to discrimination under any program or activity receiving federal financial assistance. Related statutes have broadened the grounds to include age, low income and disability.

The Civil Rights Restoration Act of 1987 also broadened the scope of Title VI coverage by expanding the definition of terms “programs or activities” to include all programs or activities of Federal Aid recipients, sub-recipients, and contractors, whether such programs and activities are federally assisted or not.

The City of Bradenton values diversity and welcomes input from all interested parties, regardless of cultural identity, background or income level. Moreover, the City believes that the best programs and services result from careful consideration of the needs of all of its communities. Therefore, it is the expressed policy of the City that no person shall be excluded from participation in, be denied the benefits of, or be otherwise subjected to discrimination under, any program or activity receiving federal funds on the grounds of race, color, national origin, sex, age, disability or income. The City of Bradenton is committed to ensure that the needs of all of its citizens are equitably represented in transportation programs, services and activities.
