FORM REQUIREMENT
Substantial Improvement/Substantial Damage (SI/SD) Application Required for New, Alteration or Accessory Structure permit types for property located in the Special Flood Area.
Non-Conversion Agreement For flood prone enclosures below the design flood elevation.
Erosion/Best Management Practices Required with Permit Application for any construction that can possibly create erosion or sediments to leave a property.
Asbestos Required with Permit Application for: Commercial construction including alterations, siding, roof, mechanical, and plumbing; and for Residential construction involving structures built prior to 1980 that include alterations, siding, plumbing, and building relocation. 
Fence Affidavit Required with Permit Application for all fence permits (Required to be recorded prior to submittal). 
Demolition Affidavit Required with Permit Application for demolition. 
Driveway Affidavit Required with Permit Application for driveway/sidewalk and new residential.
Swimming Pool/Spa Safety Certification Required with Permit Application for pools and spas.
Impact Fee Application Required with Permit Application for New or Alteration permit types.
Impervious Coverage Calculation Required with Permit Application for any addition to land that increases impervious area including, but not limited to new construction, additions, driveways, sidewalks, pool decks and slabs. 
Re-Roof Affidavit Contractor is required to post on the jobsite at the appropriate inspection.

Owner as Contractor Required with Permit Application for Property Owner acting as own contractor.
Notice of Commencement Required to be Recorded and on the jobsite for the 1st inspection, when the cost of construction is $5,000 or more except for AC Change Outs when the cost is $15,000 or more. 
Fire Application Required application for Fire Department Review: Fire Alarm, Bidirectional Antenna, Control Burn, Hydrant Flow Data, Fire Pump, Fire Sprinkler System, Fire Suppression System, Fire System Underground.
Tree Removal Application Permission for removal of live, dead or dying trees.
Plan Amendment Submit when new plans need to be reviewed due to Plans Reviewer request or changes by contractor requested.
Contractor Registration State Certified Contractors not previously registered with the City of Bradenton.
Subcontractor Verification Form Required to be uploaded to Permit if a Subcontractor is added or changed after Permit Application submittal.
Private Provider Registration Private Providers will register their license so they can be listed on permits.  Notice of Intent, Inspection Reports, and Certificate of Compliance are emailed to plansreview@bradentonfl.gov.
Private Provider Application Documents Required during permitting if the property owner or contractor is choosing to use a private provider for inspections or plan review.
Authorized Agent Form For Representation of a Contractor; submitted with Contractor Registration, Renewal or when changes in staff occur.
Tree Contractor Registration Individuals owning a Tree Removal Business wanting to do work in the City of Bradenton.
